Awareness about Tinnitus among General Public of Himachal Pradesh With Regard to Its Causes, Effects and Management

Abstract

Background: Tinnitus is the perception of a ringing, buzzing, hissing, or roaring sound in one or both ears. In Himachal Pradesh, a very few studies have been carried out to determine how well the general public understands the Tinnitus. The present study aimed to describe the awareness about tinnitus among general public of Himachal Pradesh with regard to its causes, effects and management. Material and Methods: This cross-sectional survey of residents of the state of Himachal Pradesh was carried out between October 2022 to December 2022 utilizing a Google form. Up until 400 replies were gathered, the Google form was distributed around state citizens in both rural and urban areas for responses via online means like email and social media platforms like WhatsApp groups, Facebook, Instagram, and LinkedIn. We collected data on their socio-demographic traits and awareness about tinnitus. Epi info v7 software was used to evaluate the data using the necessary statistical tests. Results: A total of 400 respondents took part in the study, including 145 (36.25%) from urban areas and 255 (63.75%) from rural areas. Among the majority of participants, 260 (65.0%) of the participants were male, 180 (45%) were between the ages of 18 and 30, 154 (38.5%) had a graduate degree, 279 (69.75%) were in the employment, 242 (60.5%) were married, and 384 (96%) were Hindu. In the present study 12% (48) participants had very good knowledge (16-20 marks) towards Tinnitus, 18.75% (75) had good knowledge (12-15 marks), 36.75% (147) had fair knowledge (8-11 marks) and 32.5% (130) having poor knowledge (<8 marks). Conclusion: Many survey respondents still unaware about causes, effects and management of tinnitus. Hearing health promotion programmes should include information on tinnitus and its management.

INTRODUCTION

The sensation of a ringing, buzzing, hissing, or roaring sound in one or both ears is known as tinnitus. It might be constant or sporadic, pulsating or steady, and its intensity can range from a gentle buzz to a loud ring. Men are more likely than women to experience tinnitus, and as people age, its frequency increases. Tinnitus can be a nuisance, but it typically does not indicate a significant issue. Regardless of an individual's age, gender, ethnicity, or socioeconomic background, its intrusive nature can interfere with their way of life [1-2].

 

Tinnitus can be brought on by illness, certain medications, head and neck injuries, surgery, and exposure to loud noises during events like raves and concerts. According to several research, exposure to overly loud noise through personal listening devices (PLDs), leisure activities, or going to nightclubs has been identified as the primary cause of tinnitus [2-3].

 

However, it appears that despite tinnitus becoming more common, there is still a lack of understanding regarding the effects of loud noise on the ears and the use of hearing protection devices (HPDs). Therefore, attempting to modify people's attitudes may be beneficial [1,4].

 

Tinnitus can have a variety of affects on a person's everyday life, including making it difficult to fall asleep, making them feel melancholy, irritable, interfering with social activities, having trouble hearing, feeling confused, and disrupting their employment and daily activities. Tinnitus can have detrimental consequences on a person's daily life, including social interactions, academic success, and career advancement. These effects can include difficulty falling asleep, difficulty concentrating, and psychological issues. Some research has hypothesized a connection between young individuals with tinnitus who also experience anxiety, depression, and suicide [1,5].

 

Due to its multiple possible manifestations, tinnitus can be managed in a variety of ways, depending on its underlying cause. Despite the fact that there are various tinnitus treatment techniques, most patients may not seek assistance because they are unaware of these techniques. This demonstrates the necessity for young adults to have more knowledge about tinnitus [1,6].

 

Numerous researches have been conducted on Tinnitus in various regions of India, but very few of these studies have been conducted in the state of Himachal Pradesh. The goal of the study was to gauge people's familiarity with Tinnitus so that a more culturally appropriate education plan could be developed as a health promotion intervention.

 

Objectives of the Study

To evaluate the awareness about tinnitus among general public of Himachal Pradesh with regard to its causes, effects and management

MATERIALS AND METHODS

Research Methodology

 

  • Research Approach: Descriptive

  • Research Design: Cross-sectional survey design

  • Study Area:  Whole state of Himachal Pradesh

  • Study Duration: between October 2022 to December 2022

  • Study Population: All adults above 18 years of age who were staying in the Himachal Pradesh for 12 months or more

  • Sample Size: 400 Adults assuming 50% have adequate knowledge regarding Tinnitus, 5% absolute error, 95% confidence level, and 5% non-response rate

  • Study Tool: A google form questionnaire consisting of questions regarding socio-demography, causes, effects and management of Tinnitus was created. The questionnaire was initially pre-tested on a small number of people to identify any difficulty in understanding by the respondents

 

Description of Tool

 

  • Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, gender, marital status, religion, employment, education and many more

  • Questionnaire: The questionnaire contains 20 structured questions regarding knowledge about causes, effects and management of Tinnitus having three options i.e. Yes, No and Don’t Know.  The participants have to choose right one. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero in each category. Scoring was done on the basis of marks as>80% (16-20) = very good, 60-79% (12-15) = Good, 41-59% (8-11) = Fair,<40% (<8) = poor

  • Validity of Tool: by the experts in this field

  • Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated via online modes like e-mail and social media platforms like WhatsApp groups, Facebook, Instagram and LinkedIn in both rural and urban area of Himachal Pradesh till the 400 responses were collected

  • Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage

  • Ethical Considerations: Participants confidentiality and anonymity was maintained. 

RESULTS

The goal of the current study was to assess general community knowledge regarding causes, effects and management of Tinnitus in Himachal Pradesh through a non-experimental descriptive survey.

A total of 400 respondents including 145 (36.25%) were from urban area and 255(63.75%) were from rural area were participated in the study. Among the participants of the majority 260(65.0%) were males, 180(45%) were between 18-30 years, 154(38.5%) were Graduate, 279 (69.75%) were employee, 242(60.5%) were married and 384 (96%) were Hindu.

DISCUSSION

In this study, majority of participants did not know about Tinnitus, its causes, effects and management.  Similarly, in the study done by Vedika Bagwandin et al. [1], the majority of participants (69.86%) were not aware of the existence of tinnitus and most of the participants did not know about the causes or effects of tinnitus.

 

It is obvious that people today engage in loud sounds when engaging in leisure activities. These individuals are said to overdo their music listening for societal norms and expectations-related reasons. Therefore, these people are at risk of developing tinnitus while engaging in leisure activities, using PLDs, and listening to loud music from other sources. Therefore, it has been advised that people be made aware of the risks associated with prolonged exposure to loud noise in nightclubs, parties, and concerts, as well as the benefits of utilising hearing protection devices (HPDs) [1,7-8].

 

People may not always be aware that there are management measures for tinnitus and may not seek help as a result. These strategies include the use of hearing aids, counselling, acoustic therapy, relaxation techniques, pharmaceutical, and surgical approaches. Additionally, it is argued that because there is no associated discomfort with tinnitus, people may not seek treatment because they do not view it as a serious health issue [1,9].

 

It is obvious that individuals need to be made better aware of what tinnitus is, its causes, and its effects. However, rather than concentrating on how tinnitus generally occurs, it might be more advantageous to enlighten people about the specific effects of tinnitus on their lives. People may be more likely to take precautions if they are aware of how something impacts them specifically. According to recent studies, people should be exposed to tinnitus health promotion programmes in a variety of methods to increase their likelihood of following guidance [1,10].

 

Limitations of the Study 

The present study has certain limitations such as cross sectional in nature, and small sample size that made difficult to generalize the findings. Future research should be focused on larger sample and qualitative studies such as focus group interviews to identify barriers to promote appropriate knowledge and practices about tinnitus.

CONCLUSION

People need to be aware of tinnitus in order to protect themselves from its causes and know where to go for assistance if they experience it. It was clear that most participants in this survey had little to no knowledge about tinnitus or its causes. Health promotion programmes for tinnitus are crucial because they will make sure that people minimize their exposure to the condition, which may inadvertently result in a long-term reduction in tinnitus and its adverse effects. Additionally, by educating individuals on the proper management of tinnitus, those who suffer from the condition will be aware of where to go for assistance. This will have a positive effect on their career and overall quality of life.
