Assessment of Knowledge Regarding Hematologic Disorders among the Patients Attending Outpatient Department of Civil Hospital, Nurpur, Himachal Pradesh

Abstract

Background: Hematologic disorders encompass a broad range of conditions that affect the blood and its components. These disorders can have significant implications for patients' overall health and well-being. The current study aims to assess the knowledge and understanding of hematologic disorders among patients attending the outpatient department of Civil Hospital, Nurpur. Material and Methods: This cross sectional study was conducted between January 2023-April 2023 among patients attending civil hospital Nurpur, Himachal Pradesh and selected by convenience sampling technique. Data was collected through face-to-face interview using a semi-structured questionnaire. Data was analyzed using epi info v7 software using appropriate statistical tests. Results: A total of 400 respondents including 210 (52.5%) male patients and 190 (47.5%) female patients were participated in the study. Among the total, 123 (30.75%) were from urban area and 277 (69.25%) were from rural area.  In the present study 14.5% (58)  participants had very good knowledge (16-20 marks) towards Hematologic disorders, 30.25% (121) had good knowledge (12-15 marks), 34.25% (137) had fair knowledge  (8-11 marks) and 21% (84) having poor knowledge (<8 marks). Conclusion: The findings of our study concluded that there are significant gaps in understanding regarding Hematologic disorders, indicating a need for improved patient education and awareness programs. Addressing the identified knowledge gaps and misconceptions through targeted interventions can improve patient outcomes, promote early detection, and enhance adherence to treatment regimens.

INTRODUCTION

Hematologic disorders encompass a broad range of conditions that affect the blood and its components. These disorders can have significant implications for patients' overall health and well-being.  Hematologic disorders involve abnormalities in the cellular components of the blood, such as red blood cells, white blood cells, and platelets. Examples of hematologic disorders include anemia, leukemia, lymphoma, thrombocytopenia, and hemophilia. These conditions can arise due to various factors, including genetic predisposition, autoimmune processes, infections, and exposure to environmental toxins [1-2].

 

Adequate knowledge about hematologic disorders is essential for patients to understand the nature of their condition, recognize potential symptoms, adhere to treatment regimens, and make informed decisions about their healthcare. However, limited awareness may lead to delayed diagnosis, inappropriate self-management, and suboptimal treatment outcomes. Therefore, assessing patients' knowledge regarding hematologic disorders is vital for identifying knowledge gaps and designing targeted educational interventions [2-5].

 

Evaluating the level of knowledge among patients is crucial for enhancing awareness, early detection, and appropriate management of hematologic disorders. Improving patients' knowledge and understanding of hematologic disorders can lead to better self-care practices, early detection of symptoms, and timely medical consultation. Moreover, it can contribute to increased patient empowerment, better treatment adherence, and improved overall outcomes for individuals living with hematologic disorders [3-6].

 

However, limited research has been conducted to assess the knowledge and understanding of hematologic disorders in Himachal Pradesh. The current study aims to assess the knowledge and understanding of hematologic disorders among patients attending the outpatient department of Civil Hospital, Nurpur.

 

By evaluating patients' knowledge and perceptions regarding hematologic disorders, this study will provide valuable insights into the existing level of awareness and identify areas for improvement. The findings will help identify knowledge gaps, misconceptions and educational needs among patients. This information can guide the development of targeted educational programs and support healthcare professionals in providing accurate information and counseling to patients.

 

Objectives of the Study

To evaluate the level of knowledge and perceptions regarding Hematologic disorders among patients attending the outpatient department of Civil Hospital, Nurpur.

MATERIALS AND METHODS

Research Methodology

 

  • Research Approach: Descriptive

  • Research Design: Hospital based cross-sectional survey design

  • Setting of the Study: OPD in Civil hospital Nurpur, Himachal Pradesh

  • Study Duration: Between January 2023- April 2023

  • Study Population:  Patients aged above 30 years came to OPD 

  • Sample Size: 400 patients, assuming 50% have adequate knowledge regarding Hematologic disorders, 5% absolute error, 95% confidence level, and 5% non-response rate

  • Sampling Technique: Convenience  sampling technique

  • Sampling Criteria: First five patients who came to OPD were selected everyday till the completion of sample size after explaining the purpose of the study. Informed consent was taken from them and confidentiality  of the selected  participants was also maintained

  • Inclusive Criteria: Patients who were willing to participate in the study

  • Exclusion Criteria:  Patients who were not willing to participate in the study

  • Description of Tool: 

  • Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, marital status, religion, employment, education, family’s monthly income etc.

  • Questionnaire: The questionnaire contains 20 structured knowledge related questions having three options (yes, no, don’t know).  The participants have to choose right one. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as >80% (16-20) = very good, 60-79% (12-15) = Good, 41-59% (8-11) = Fair, <40% (<8) = poor

  • Validity of Tool: by the experts in this field

  • Data collection by  under the guidance of supervisors and patients were given 30 minutes time to complete that questionnaire and collected at the end of the prescribed time

  • Data analysis with Epi Info V7 Software with appropriate statistical test in terms of frequencies, percentage

RESULTS

The goal of the current study was to assess the awareness about Hematologic disorders among patients attending outpatient department of civil hospital, Nurpur, Himachal Pradesh.

 

A total of 400 respondents including 210 (52.5%) male patients and 190 (47.5%) female patients were participated in the study. Among the total, 123 (30.75%) were from urban area and 277 (69.25%) were from rural area.

DISCUSSION

The findings of our study provide insights into the level of knowledge and awareness regarding hematologic disorders among patients attending the outpatient department of Civil Hospital, Nurpur. It was observed that there are significant gaps in understanding, indicating a need for improved patient education and awareness programs. Limited knowledge about hematologic disorders can lead to delayed diagnosis, inadequate self-management, and suboptimal treatment outcomes.

        

Our study assessed patients' knowledge of common hematologic disorders such as anemia, leukemia, lymphoma, thrombocytopenia, and hemophilia. It was found that patients had varying levels of awareness about these disorders, with some demonstrating a good understanding while others had limited knowledge. Identifying specific areas of low awareness can help tailor educational initiatives and improve patient understanding of these conditions.

 

The study examined patients' knowledge of risk factors and symptoms associated with hematologic disorders. It was observed that patients had varying levels of understanding regarding the factors that contribute to the development of these disorders, such as genetic predisposition, autoimmune processes, infections, and environmental factors. Similarly, patients demonstrated different levels of awareness of the symptoms that may indicate the presence of a hematologic disorder. Identifying and addressing these knowledge gaps is crucial for early detection and timely medical intervention.

 

Our study also assessed patients' understanding of the available treatment options for hematologic disorders. It was found that patients had varying levels of knowledge regarding treatment modalities, including medication, transfusions, chemotherapy, and stem cell transplantation. Some patients may have misconceptions or incomplete information about the efficacy and potential side effects of these treatments. Educating patients about treatment options and their benefits can empower them to make informed decisions about their healthcare.

 

The findings of this study highlight the importance of patient education and awareness programs regarding hematologic disorders. Addressing the identified knowledge gaps and misconceptions through targeted interventions can improve patient outcomes, promote early detection, and enhance adherence to treatment regimens. Healthcare professionals should play an active role in providing accurate and evidence-based information to patients, addressing their concerns, and promoting preventive measures and appropriate treatment options for hematologic disorders.

 

Limitations and Future Directions

This study has certain limitations. The findings are based on self-reported data and may be subject to recall bias or social desirability bias. Additionally, the study was conducted in a specific healthcare setting in Nurpur, Himachal Pradesh, which may limit the generalizability of the findings to other regions or settings. Future research should include larger sample sizes from diverse populations and multiple healthcare facilities to obtain a comprehensive understanding of the knowledge and perceptions regarding hematologic disorders among the general public.

CONCLUSION

In conclusion, our study underscores the importance of assessing the knowledge and awareness of hematologic disorders among patients attending the outpatient department of Civil Hospital, Nurpur. Addressing the identified knowledge gaps through targeted educational interventions can improve patient outcomes, facilitate early detection, and promote appropriate management of hematologic disorders.
